Blood (SST)

The pituitary signal. Context for interpreting thyroid hormone levels.

Reference range: 0.5 to 2.5 mIU/L optimal

Free T4
Blood (SST)

The storage form. Most thyroid medication replaces this.

Reference range: 12 to 22 pmol/L

Free T3
Blood (SST)

The biologically active hormone your cells use.

Reference range: 3.5 to 6.5 pmol/L

Reverse T3
Blood (SST)

The inactive metabolite. Elevated rT3 blocks T3 at the receptor level and explains persistent hypothyroid symptoms despite normal other markers. The T3 to rT3 ratio is more informative than rT3 alone.

Reference range: Under 250 pg/mL, ratio to T3 matters more

TPO Antibodies

Catches Hashimoto's autoimmune activity.

Reference range: Under 35 kIU/L

Tg Antibodies
Blood (SST)

Adds specificity to autoimmune detection.

Reference range: Under 40 kU/L

T3 : rT3 Ratio
Calculated

One of the most useful ratios in complex thyroid cases. Low ratio indicates T4 is being shunted to inactive rT3, common in stress, chronic illness, and prolonged calorie restriction.

Reference range: Above 20 preferred
